What every intensivist should know about monitoring respiratory drive and effort

The clinical challenge is not merely to avoid the extremes of overand under-assistance, but to identify the physiological window in which inspiratory effort maintains diaphragmatic conditioning without exposing the lung to injurious transpulmonary pressures.

There is a need to monitor the patient-specific balance between respiratory drive, inspiratory effort, and mechanical power to tailor ventilatory support within an appro￾priate physiological window.
